Falls are the leading cause of injury in senior care. Every second, an older adult falls in the U.S. and, despite caregivers’ best efforts, half of skilled nursing facility residents experience a fall each year. This has a huge toll on both human lives and how communities operate.
Falls derail health, drive hospitalizations, reduce occupancy, trigger move-outs, and expose communities to legal and financial risk. In assisted living, the annual direct cost of falls can reach $380,000 per community—and up to a staggering $712,000 for larger enterprise communities. But it doesn’t have to be this way. At Sage, we believe there’s a way to do things differently in senior living. A way to not only respond faster, but to proactively prevent more emergencies, like falls, from happening at all.
That’s why we’re proud to introduce Sage Detect, a new, fully integrated AI-powered monitoring system that transforms how senior living communities detect emergencies and support resident wellness. “Caregivers are under pressure to do more with less,” said Sage co-founder and CEO Raj Mehra. “Sage Detect empowers them with real-time alerts and intelligent, granular insights that are fully integrated into their current workflows. The result is faster responses, improved staff efficiency, and a more proactive approach to care.”
Sage Detect is embedded seamlessly into the Sage operating system, which communities around the country already use to triage alerts, check in with residents, manage care, and document interventions. Sage collects and interprets data to help teams improve outcomes, identify financial opportunities, optimize staffing, and streamline operations to empower smarter decision-making across every level of a senior living community.
Now, Sage Detect layers on AI-powered video to detect falls and other potential emergencies. When an incident is detected, caregivers get an instant alert in the Sage app. From there, they can claim the event, confirm the incident, communicate with team members, and document care in mere seconds. This real-time, closed-loop system dramatically reduces response times during emergencies.
Beyond emergency detection, Sage Detect offers staff risk alerts (e.g. trip hazards) and up-to-date visibility into resident activity and well-being, helping staff prioritize care. Sage Detect surfaces crucial wellness trends, like sleep quality, activity levels, bathroom frequency, and more. These indicators often serve as early warning signs, enabling caregivers to intervene earlier and more effectively, before the situation escalates and the resident is even at risk.
The depth and accuracy of Sage Detect’s proprietary AI sets this system apart from legacy solutions. Unlike other systems that use limited fall classification algorithms or surface-level dashboards, Sage Detect is powered by the world’s most advanced vision-language AI models. It interprets a wide range of resident behaviors, yielding high-quality data that it then translates into specific, actionable insights for care teams. This level of detail supports better care plans, more targeted staff training, and improved clinical outcomes.
With any new technology, trust is paramount. That’s why Sage Detect was built with privacy and security at its core. Our monitoring is fully automated by AI with strict privacy controls. Unlike other fall detection systems, humans never monitor video feeds. When footage surrounding an incident is necessary for quality assurance or clinical needs, only pre-approved staff can access it, and images can be obfuscated for an added layer of privacy. Our system is HIPAA-compliant and all video is automatically deleted after 30 days.
During development, Sage collaborated with Agemark, a national senior living provider, to shape Sage Detect into a caregiver-first product. Agemark also became the first to pilot Sage Detect. “We’ve had great success with Sage throughout Agemark communities and we’re excited about this evolution and the power of Sage Detect to accelerate response times, improve care planning, and boost operational efficiency,” said Michael Pittore, co-CEO at Agemark. “Its targeted AI-driven insights will help our teams identify condition changes earlier, respond more effectively, and ultimately deliver safer, more proactive care, while maintaining the privacy and security that our residents deserve.” (Listen to Mehra and Pittore discuss this partnership on the Bridge the Gap podcast.)
Sage Detect is more than a fall detection system. It’s the next generation of intelligent proactive care, giving staff the right information at the right time and empowering them to make faster, smarter decisions that improve resident safety, family satisfaction, and community performance.
With Sage, communities can move away from rigid tech stacks, inefficient workflows, and outdated fall detection and nurse call systems. “With the addition of Sage Detect to Sage’s already successful operating system, we’re adding a layer of monitoring that gives caregivers more support and peace of mind—without any headache,” Mehra said. Better care starts with more proactive insights—and those insights begin with Sage Detect.
